Measure person and system outcomes that assess relationships beyond health state with a priority to emphasize benefit over deficit (e.g., vulnerability and disease state).
Leverage existing infrastructure
Reduce burden to patients and providers by prioritizing the use of existing data and data collection systems, create interoperable structures, and generate consistent data to advance shared knowledge.
Create person-centered and community-valued insight
Individuals, providers, and community stakeholders participate in cobuilding and benefit from information collected to guide action.
Mission aligned and partnership driven
Use measures valued by and relevant to multisystem partners, creating shared responsibility to measure and report to demonstrate the complex relationship of FIM benefits.
Inclusive and equity driven
All measures support knowledge creation that address — not exacerbate — disparities between and within communities.
Reasonable in expectation
Measurement is balanced and reasonable, comparable to other health interventions, in relation to anticipated impact to affirm and demonstrate positive effect.
