This report describes how communities can use community health improvement processes (CHIPs) to promote health and prevent disease. The report recommends CHIPs as a tool for:
- Identifying and prioritizing population-level health needs
- Developing interventions to address those needs
- Monitoring performance and outcomes of interventions
The committee encourages using performance monitoring as part of the CHIP process to:
- Identify and measure processes and outcomes
- Assess intervention efficacy
- Hold stakeholders accountable
Additionally, the committee recommends tools for helping communities develop their own performance indicators.
