Between October 18–21, this website will move to a new web address (from health.gov to odphp.health.gov). During that time, some functions might not work as expected. We appreciate your patience and understanding as we’re working to make this transition as smooth as possible.

Healthy People in States and Territories

Healthy People State and Territorial Coordinators

The Healthy People State and Territorial Coordinators make Healthy People happen every single day across the United States. Each state or territory has a Healthy People Coordinator who serves as a liaison with the Office of Disease Prevention and Health Promotion (ODPHP). ODPHP works with Coordinators to identify areas of alignment in their work and the Healthy People 2030 goals and objectives. They also collaborate with the HHS Office of the Assistant Secretary for Health (OASH) Regional Offices. For more information about disease prevention and health promotion efforts in your state or territory, check out the health improvement plans below.

State and Territorial Health Improvement Plans

Healthy People has linked to the most recent available version of each state’s and territory’s improvement plan. Explore your state’s or territory’s improvement plan below to learn how it addresses the Healthy People 2030 Leading Health Indicators (LHIs) affecting your community. Plan implementation may not align with the Healthy People 10-year cycle and may still be in progress, evolve over the decade, or be complete. Please check back regularly for updates on available health improvement plan information.

Noted LHIs reflect Healthy People’s assessment of the health improvement plans and may not represent all areas of alignment with Healthy People 2030.

Wisconsin Department of Health Services: Centering Health Equity in State Health Improvement Planning

Map of States and Territories with OASH Region numbers

Healthy People 2030 goals and objectives guide the Wisconsin Department of Health Services (DHS) throughout the development and implementation of its State Health Improvement Plan. DHS embraces its mission to “protect and promote the health and safety of the people of Wisconsin” and works toward a vision of “everyone living their best life.” Unlike other states with a standalone public health agency, DHS includes public health under its umbrella of programs and services. This brings with it both benefits and challenges.
