This resource from Million Hearts® 2022 provides a list of process improvements for increasing cardiac rehabilitation uptake in eligible patient populations. Cardiac rehabilitation is an evidence-based prevention program for improving cardiovascular health after a cardiac-related event or procedure. This change package provides:

  • Guidance on identifying changes to guide process improvement
  • Evidence-based and practice-based tools
  • Resources for implementing the tools

Quality improvement teams can use the interventions, strategies, and evidence-based tools in this package to improve cardiac care and increase the use of cardiac rehabilitation programs.
