Summary
People with end-stage kidney disease (ESKD) need dialysis or a kidney transplant to survive. Preventing ESKD requires monitoring and treating chronic kidney disease (CKD), diagnosing and treating people with acute kidney injury, and managing populations at risk for CKD — like people with diabetes or high blood pressure. Population health and team-based approaches to diabetes and kidney care are linked to fewer new ESKD cases in some groups.
